Please have your insurance card with you at the time of your visit. 

You are responsible for any co-pay, co-insurance, and/or deductible amounts due at the time of the vist.  If we are not contracted with your insurance, we are still happy to treat your child, however, payment for service in full will be due at the time the services were rendered.  We can provide, at your request, our best estimate of charges prior to your appointment based on the reason for your child's visit.

Please be aware that if your child is covered under any AHCCCS plans, we are unable to see them as a self-pay patient.  AHCCCS rules do not allow for our providers to treat patients on a cash-pay basis.

• We do not partcipate in any Cost Share programs. We are still able to see your child on a self-pay basis.  After your visti, we will give you the reciept so that you can submit your paperwork to your insurance for reimbursement. Examples of these plans include: OneShare Health, UHSM, Christian Care Insurances, Solidarity Health Share, Christian Healthcare Ministries, Medi-Share and Samaritian Ministries.
